Billing
Community Health Center accepts patient payments through:
- Personal check, cash, Visa, MasterCard, debit.
- We accept many private insurance and will bill your insurance.
- Our billing department will bill you for any balance that is your responsibility.
- We accept
- Oregon Health Plan open medical cards for all new and established patients, any age.
- Oregon Health Plan - Care Oregon for all new and established patients, any age.
- ODS - Mid Rogue IPA (if assigned to us)
- Medicare
Community Health Center will work in partnership with you
- Quality of care is Community Health Center's first priority.
- If uninsured, we ask that you make a minimum payment towards the bill at the time of each visit. Any remaining balance can be paid in monthly payments.
- If unable to make a minimum or monthly payment, you need to talk with us. Arrangements can be made for an affordable payment plan. If no effort is made to pay your bill, and we do not hear from you, you may be refused further services.
- Patients who pay on their account help ensure that affordable health care will be available in the future, when they or their family needs it.
Payment for Services
- Sliding Fee Scale
- If a patient has a limited income, they may qualify for discounted services, depending on their household income and the number of persons living on that income.
- Prenatal care services are available at our White City office. Women who are uninsured and do not qualify for the Oregon Health Plan may qualify for discounted prenatal care.
- Oregon Health Plan (OHP) offers health care to its members at little or no cost.

Care Oregon - We accept Oregon Health Plan patients enrolled in Care Oregon.
- Third Party Insurances
If patient's have health insurance, we will bill their insurance company for them. Patients are responsible for all co-payments and any unpaid balance at the time of their visit.
- Current Insurance Information
It is very important that we have our patient's correct insurance information in order to process your charges in a timely manner.
