Community Health Center Serves:

  • Medically uninsured residents and their families who have limited ability to pay for timely health care.
  • People who have a job without employer sponsored insurance and make just a little too much money to qualify for the Oregon Health Plan.
  • People who have language, cultural or other non-financial barriers to obtaining health care.

Primary health care is basic medical care encompassing curative and preventative medicine, health promotion and maintenance, education, nutrition and continuing care evaluation and management. Provided on an out-patient basis, primary care services as the critical entry point to the health care delivery system. Services include:

  • Diagnosis & treatment of illness
  • Screening & disease prevention
  • Well child exams & Immunizations
  • Women's health exams
  • Physical examinations
  • Management of chronic illness
  • Health education
  • Prenatal care (White City Clinic)
  • Specialty clinics

Ancillary services include:

  • Laboratory services (by referral)
  • Specialty care (by referral)
  • Radiology (by referral)
  • Patient education
  • Prescription medication (grant program/by referral)
  • Medication refill
    • PATIENTS NEED TO PLAN AHEAD and not wait until they are out of medication to request a refill.
    • PATIENTS NEED TO CALL THEIR PHARMACY at least 3 days before they are out of their medication to request a refill. The pharmacist will contact their primary care provider.
    • PATIENTS SHOULD NOT JUST DROP IN to request a refill. Before medication can be refilled, the patient's primary care provider must carefully review the request. This may take 48 to 72 hours.
